The Patient Centered Medical Home and Specialty Practice Models are nationally-recognized frameworks for achieving excellence and high-performance among primary care and specialty practices. The Kentucky REC offers two types of services: one for primary care practices and one for specialty practices.

The Patient-Centered Medical Home (PCMH) recognition program is a practice transformation model that helps practices improve in areas of patient-centeredness, access, communication, care coordination, quality, and efficiency.
Patient-Centered Specialty Practice
Patient-Centered Specialty Practice (PCSP) is an innovative program for improving specialty care that was built on the success of the Patient-Centered Medical Home standards and makes recognition available to specialty practices.
