khieKentucky Health Information Exchange (KHIE) Important Notice to Providers Attesting to Meaningful Use:

As the Public Health Authority for the state of Kentucky for public health measures within the CMS EHR Incentive Program, KHIE would like to remind you that a provider must have a signed Participation Agreement and signed addendum(s) on file with KHIE for each public health objective selected for attestation.

In an effort to assist you in meeting the CMS EHR Incentive Program Year 2015 deadline and be deemed a Meaningful User, KHIE is providing the following notice:

    • All requests to connect to KHIE must be received no later than December 11, 2015.
    • To be considered in KHIE’s queue during your selected attestation reporting period, KHIE must receive a completed KHIE Intake Form. This form is available on KHIE’s website or from the Outreach Coordinator assigned to your region of the state. To view this information, please click here.
  • Providers who complete and return the KHIE Intake Form by the December 11 deadline will be processed into the system. KHIE will forward the KHIE Participation Agreement and addendums to you for signature. The Participation Agreement and applicable addendums must be returned to KHIE byDecember 30, 2015 (State Offices are closed on December 31).
  • Requests received after December 11, 2015 will be processed after January 4, 2016.

If you are already connected to KHIE, please ensure that all of your public health addendums have been signed and returned by December 30, 2015.
